This is the one that comes with the swivel. Buy it if you do not already own a push-button QD swivel — it saves you a second order.

Daniel Defense makes two sling mounts that are easy to confuse. This part, 03-021-30029, clamps to Picatinny rail and ships with a Heavy Duty Push Button Quick Detach Sling Swivel in the box. The other, the MFR Rail M-LOK QD Sling Mount, 03-021-15136, attaches to M-LOK on Daniel Defense rails and does not include a swivel. Same brand, same job, different interface and different contents.

The mount itself lets you put the swivel anywhere along a rail system rather than at whatever fixed point the rifle came with, and it is shaped to stop the sling tangling on itself. Installation uses Daniel Defense's patented Rock & Lock one-piece mounting system, which simplifies fitting on MIL-STD-1913 rails. It is machined from 6061-T6 aluminium with a Military Specification Hard Coat Type III anodised finish, weighs 0.5 oz (14 g) and is made in the USA. The supplied swivel has a durable 1.25" (32 mm) loop and accepts sling straps up to 1.25".
